How do I define a usage / rate plan ? The documentation shows screenshots with “additional prefix/field” section, which doesn’t exist in production.

You can add addition custom fields to almost every entity in BillRun.
You can do so from settings->custom fields. In the screenshots you see a custom field with the name “prefix”.
To create a plan you can do it from menu at postpaid->plans.
